Jeb Hornsby, MD
Dr. Hornsby has been practicing in Hartselle since 2010. He is board certified in family medicine as well as facial aesthetics. He attended the University of Alabama for undergrad, UAB for medical school, and completed Family Practice Residency in Tuscaloosa. He's married to Jennifer and they have 3 children.
Jessica Smith, CRNP
Jessica graduated from The University of Alabama School of Nursing in 2012 and then followed up with UAB for NP school. She's been a family practice nurse practioner for 8 years and has worked with Dr. Hornsby since 2019. She is married to Jordan and they have 2 small children.
Cheyenne Crowe, CMA (AAMA), CFB
Not only is Cheyenne the clinic's Office Manager, but she's also a Certified Medical Assistant and Biller. Cheyenne has worked with Dr. Hornsby since 2012. She's married to Tyler and they have 2 boys.
Hannah Wiley, CMA (AAMA)
Hannah is a Certified Medical Assistant at our clinic. She is engaged to Dustin and will be getting married later this year. Hannah worked with Dr. Hornsby since 2016.
Brianna Albrecht, CMA (AAMA)
Bri is a Certified Medical Assistant and has worked with Dr. Hornsby for over 5 years. She is married to Steven and they have one little boy.
Ashton Anders, RT(R)
Ashton is an X-Ray tech and has worked with Dr. Hornsby since 2022. She is married to Charlie and they have 2 small children.
Meagan Oliver
Meagan is cross trained for many positions in our clinic, but is currently serving in reception. She is married to Jesse and they have one little boy. Meagan has worked with Dr. Hornsby since 2015.
Jessie Kent
Jessie is one of our amazing receptionists and has worked with Dr. Hornsby since 2022. Jessie is married to Trey and they have 2 small children.
Jennifer Hornsby
Jennifer is our Director of Digital Marketing. She handles all of our social media, advertising, and website maintenance. She is married to Dr. Hornsby and they have 3 teenage children. Jennifer also owns a travel agency, Kingdom Elite Travel, and homeschools their youngest child.
